For Online Ordering:

• Please use only ONE form per child.
• Please enter quantity next to item you would like to order.
• Your total will accumulate at the bottom
• Print a copy of your confirmation for your records

For Paper Ordering:

• Please print clearly and use ONE form per child.
• Please enter quantity and dollar amount next to item you would like to order.
• Enter total at the bottom.
• Make a copy for your records and return the original with check made payable to SSDS to the Business Office at 125 Wells Avenue.
